New Patient Registration

Registering with Mythe Medical Practice

Mythe Medical Practice is pleased to welcome new patients wishing to register. Before this can take place we will firstly need to confirm that you have moved into, or are living within our catchment area. You will then need to complete certain forms and provide both evidence of identity and address and medical information in accordance with our Practice Registration Policy - a copy of which can be found by clicking the link below or is available at Reception.

To complete your registration with the practice you need to come to the surgery in person and sign an NHS registration form as well as a new patient information form. 

You will also need to provide evidence of identity and address.

For added convenience you can access and fill in these forms in advance by clicking on the "Registering Online" tab above.

You will be allocated a"usual GP" but will be free to request an appointment with the doctor of your choice.

Remember to let us know, in writing or via this website, of any subsequent changes to your personal details (e.g. change of address or telephone numbers including mobile).

Medical treatment is available from the date of registration. Please contact reception for further information. We ask that if you are currently taking regular medications you book an appointment with a GP to discuss these as soon as possible.
